School Nurse

Physicals:State Required 10th grade physicals are due by the end of December. If you have not sent it in, please do so as soon as possible.

A physical form needs to be a completed physical and cannot be substituted with an AVS (after visit summary) or visit note.

A physical is required for sports annually and under MIAA guidance valid for 13 months from date of exam. 

However I love receiving physicals and keeping information up to date, so sending them in routinely is much appreciated.

 

Immunizations: 

Students are required to have meningococcal (MCV 4, Meningococcal ACWY) at age16; please provide records to the nurse's office.

Preschool

• 1-4 doses of HIB
• 4 doses of DTap
• 3 doses of Polio
• 3 doses of Hepatitis B
• 1 dose of MMR
• 1 dose of Varicella

Grades K-6

• 5 doses of DTap
• 4 doses of Polio 
• 3 doses of Hepatitis B
• 2 doses MMR 
• 2 doses Varicella

Grade 7

• 1 dose of Meningococcal Vaccine [MCV4, MenAWCY]
• 1 dose of Tdap

Grade 7-12

• 1 dose of Tdap
• 4 doses of Polio  
• 3 doses of Hepatitis B   
• 2 doses of MMR  
• 2 doses of Varicella or physician documented case of chickenpox disease  

Grade 11

• 2nd dose of Meningococcal Vaccine [MCV4] (at age 16)

*Documentation of any exemption to immunizations must be renewed annually at the beginning of the school year  

 

Medications:

If your student takes medication at school, Please remember that all medication, even over the counter medicine will need a  Medication Order Form & Parent Consent filled out. If your student is unable to swallow medications, please provide chewable tablets or liquid.

If you have a student who currently has medication at school, this includes prescribed, over the counter medication, EpiPens, Inhalers, etc. they will need to be picked up by the last day of school. Any medication not picked up by parents/guardians will be disposed.

 

Mandated Health Screenings: 

Health Screenings are done each year throughout the year until completed unless you fill out an opt out form in the beginning of the school year.

• 10th grade Vision, hearing and BMI
• 9th grade Postural screening

Nurse Dismissals: 

Students should not use their phones to contact parents for dismissal. The nurse's office is staffed with a full time nurse who will assess the students who are not feeling well. The nurse will reach out to parents/guardians if a student is ill and needs to be dismissed. 

When Should I keep my student home?

The main reasons for keeping your child home are:

• If he/she is too sick to be comfortable at school
• If he/she will spread a contagious illness/disease to another child

A student should stay at home for:

• A fever greater than 100 degrees,
• Vomiting or diarrhea within the last 24 hours
• A diagnosis of a contagious disease, (Strep throat, conjunctivitis, covid); please alert Nurse Rachael Goetz 

Attendance 

Per the Student handbook students must turn medical notes in within 10 school days of the return to school. Notes can also be emailed directly to the nurses’ office, rgoetz@dcrsd.org or ecolon@dcrsd.org 

Please reach out as early as possible for planned absences due to surgery or medical procedures so we can support and plan for the student’s return to school.

SHRHS Student Handbook

1.  All new students must have all immunizations up to date and a current physical exam (within the last year) to start school.

2.  If your child is ill please keep them home. Examples of illnesses that your child should stay home:   Fever of 100.0 or more, vomiting , diarrhea, rash of unknown origin, contagious disease (conjunctivitis, strep throat) 

3.   Please communicate with me any illness your child may have.

4.    Please do not ever send medication into school with your child. This includes cough drops.  If your child needs any medication while at school: prescription or over the counter, please contact me to make arrangements and have the proper paperwork filled out.

5.  Physical exams are required upon entry to school,  Kindergarten, 3rd grade, 7th grade and 10th grade.
